Developed two rolls of film.
Negatives look really dark.

I used caffenol stand development, which has always worked for me.

Tried re-fixing them for 10 min, but that didn't do a whole lot.

Two different cameras, so I can rule that out.

Both rolls tmax 400

Any ideas?
 
Overall it looks like fogging, though the second image looks like you have an unfixed area, so bad fixer could be a contributing cause.
Is this fresh or old film?
Loaded onto reels in a known good changing bag or known good dark space?
How old is the fixer, and does it pass a clearing test?
The light streaks in the third picture look very odd, are those reflections?
 
what kind of film was it ?
i see tmy-2, was one of them color ?
if you soak the film in reels in running water
for a while some of the coffee stain will be liberated.
i think as bdial suggested it might be a fixer-thing too ...
whenever i cross process ( color in b/w ) it gives a bluish cast
that's what it looks like but i could be way off ..
 
The fixer was mixed up last fall, I think, but I've only done a few rolls -- maybe 5 with it.

It didn't look cloudy. I'll try a new batch and see if that helps.

Would fixing again in new fixer help any?

It's a bit difficult to tell for certain but I agree with others... try fresh fixer.
