If anyone is interested in how to open the front of a Medical Nikkor 120/4-32 to clean dusty lenses or an oily aperture:

What had to be done, and would you like to explain the control electronics in the tube? There's a four-sided circuit board with discrete components inside, unbelievable!

The 120/4 Medical Nikkor came to market in 1981.

Looks like it is in nice shape. How is the flash triggered? From the diaphragm movement?

I b'lieve they're fixed aperture, could be mistaken. Effective aperture varies with magnification.

Is the small light bulb part of a lighting system?
The small light bulb is the modeling light.
As written before the dusty lenses and the oily aperture had to be cleaned.
This MEDICAL lenses have a aperture that is mechanical coupled with the magnification rate and the focus distance - the smalles magnification rate works only with f/4 at a focussing distance of about 1.5 meters, and the biggest magnification rate is at f/32 at a focussing distance of some 20 centimeters.
The MEDICAL controls the flash power via a photocell located in the bayonet that measures the light reflected from the film surface.
