Only when working closeup. Otherwise it isn't necessary.

At 1:1 (extension = FL) simply add 2EV
At 1:2 (extension = 0.5*FL) simply add 1EV

Rule applies regardless of format.

Effective aperture = (1 + m)*f, where m is the magnification and f is the physical aperture of the lens. That said, if your lens has floating elements, such as the Pentax 120mm f/4, the physical aperture changes with focus distance.
