I wonder what the final outcome was? I don't suppose we will ever hear as we haven't seen the OP since September of 2017. He may have moved to Bentonville, Arizona I suppose :D

Ask your lab whether they use dip and dunk equipment or roller transport equipment.
Yes - if its a dip and dunk machine I would say the problem is at the camera stage, if its a roller transport well they are notorious for scratching film.. I would ask this direct question

I just called them, they said roller.
theres is the most likely problem... Most of these labs use Digital Ice to make proof prints to hide this ongoing problem... I have refused to print for clients in the past who have used roller transport for BW film process and came to me for Condensor Enlarger prints.. find a lab that uses dip and dunk and this will not be a concern moving forward in your film career.
