I used to have the Epson 550 and when I would do a preview it would display all six medium format negatives as smaller thumbnails and were clearly visible. I now have the Epson 4990 and after the preview I have to outline the frame of each negative and then it would be clearly visible. Any idea why? I had a different 4990 briefly and that did the same thing which is why I returned it. I have tried uninstalling and re-installing drivers and it's the same.

In the Epsonscan preview scan window there are Normal and Thumbnail selection tabs. Normal allows selection of the framing and Thumbnail frames it automatically. Click on Thumbnail.

This is based on my V600 and V850 Epsonscan software.

I think you are in expert mode. If you go to automatic mode, it should give you what you want (if I am remembering right)...

On my 550 I had an option to select what size negative they were, but now there is no option for size.

It looks like the Epson software for the 4490 is nine years older than the software for the v550. It wouldn't surprise me if there were differences.

I was using the latest version of Epson Scan on my 4990, which is concurrent with the 4490, less than 2 years ago and it worked as you desire.

Are you using the latest software version or are you using what came on the CD?

That's how it was with the 550 but there's no option for film size with the 4490. I have uninstalled numerous times and it's stays the same. I have another computer running windows 10 and I will try loading that and see.

My Epsonscan has a sub tab in the Configuration tab window that allows selection for medium Format size such as 6x6 6x7 etc.

I used a 4990 for 15 years or so, up until recently when I replaced it with a V850 Pro. I have donated the 4990 so I can't reference the interface, but I'll say this.
Over 15 years, I made various updates with the Mac OS and the Epson software.
Whenever I scanned with the film holder (or without one), I could marquee the image however I wanted, by single frames, or one large frame to be broken up after scanning in PShop. If a large frame including rebate areas between images, I had to adjust the levels and curves control (in the Epson software) to compensate for the extreme clear area, but always had complete control over the scan.
I don't remember having to call out the neg size, just marqueed as I saw fit. Scale of the scan was likewise controlled by me.
Reading this again, I don't really understand the OP's issue, but I also don't remember tabs. It was very straightforward, as I recall.

We know that you are using it with a version of Windows.

Epsonscan provides automatic framing of the marquee to fit the film. That's what the OP wants to do. Your method works but is not what his question is. The thing with medium format, is that you have to select the size in the Configuration tab (6x6, 6x7, or etc.). You do not need to do that with 35mm film strips or other sizes since there's only one size for the non medium format film holders.
