The most recent example shows fogging around the first frame, which would suggest an issue between removing the roll from the wrapper and loading the film, before the back is closed, or does it continue through the rest of the roll?

It happens all over. No pattern of any kind and certainly not restricted to the ends of each roll. It can and has happened anywhere.
