This a Bronica SQ 120 film back. When the film is first mounted, you can wind the film manually to get on the winding reel. When we hold on the winding reel and wind manually, the winding reel (take-out reel) would not wind. There is some separation between the winding gears and the take-out reel.

So when the film back is put on the camera, film won't wind and the lens can't be cocked.

Anyone knows how this can be fixed? We would just throw it away if this were a 220 back..... But this is a 120 insert.
