In certain parts of the world (Japan only?) apparently it once was possible to buy 120 films that were half the length of the standard films. That setting was to accommodate those films.
Here in Canada I've never actually seen one of those half length 120 films.
I did read that the camera was aimed at photographers who sold portrait photos at tourist sites, for which half-length 120 rolls were made, and for which the 4-frame counter setting is intended.
Quite separate from that, you do need to work the wind-on lever twice for each frame.
You have to set the pressure plate and film selector on the camera top to 8 frames for 120. If you shoot 220 you slide over the pressure plate then select the 16 exp setting. Only in japan did them briefly make 4 exposure rolls for quick group photos at tourist sites for quick developing and printing for individual groups.
Yep the above is true from what I've found - they did indeed make a fast turnaround 4 shot roll - but I've not seen one around the north American market for sure.
I always assumed that before the 4-shot rolls and big Fuji's, group tourist photos were the reason for Rolleiflex Plate Adapters (allowing single sheet film on a Rollei.)
