What is causing these dark ugly speckles on my negatives?

Second roll of hp5 I developed with pyrocat-hd fixed with tf4 has the same issue as the first. Pan-F and Delta 3200 did not have this problem. This time, with less agitation (every two minutes). There are dark ugly speckles all over the negative. Is this just overly pronounced grain (should be smooth with pyrocat and long development time) or an error in development. Like I said, did not have this issue with Pan-F or Delta 3200

Most likely from crud in the developer. Try filtering the working solution before using the next time.
