What happened to my film?

The first three shots are bad. The last 3 are fine. Any idea what could have happened? I don't remember doing anything differently. Shot with a 612 back on a Horseman sw612p.

Edit - I wonder if I forgot to put the dark slide back in when using the ground glass adapter.

Doh- I must have. I shot a few more rolls and and they're all fine
