Comes with LTM-M adapter (from tested supplier), filter adapter ring to use widelly availible 40.5 filters, metal ugly front cap and M mount plastic cap.
Mechanically seems to be fine. Focusing in feets. Not trashed one optically. Has noticable front scratch. But not on the images.
This particular lens seems to be good example as it is described in the book. Not so sharp and dreamy at f1.5, 2 and smaller are sharp enough (for me) and shows this lens character. Nice on bw and colors negatives as well.

The reason for sale... My canadian M4-2 just told me what she wants canadian Cron on it. And those are pricy.
