While making prints recently, I grew tired of using an old lined notebook to record my printing notes, so I went into InDesign and slapped together a simple record sheet for writing down notes for my prints. I printed out about 25 of them, and put them in a 3 ring binder to keep things organized. I figured I would share the PDF file with you all so that others can take advantage of it as well, if they wish.

A good idea. I'd like to suggest that you might want to include a box for grade as well.

I usually include that in the "paper" box, because I print on both graded and VC paper. So for graded it's simply Galerie #3, and for VC I would write Adox MCC @ 4.5 or whatever.
 
Good one, all I need now is someone who will actually fill it in for me.
Me being very, very lazy concerning writting notes of my work just leave it be and see what comes out everytime I do a photo again.
