I recently purchase Polaroid back and trying to determine what model this is. It has a sticker saying modified in Japan inside.

Fuji FP100C fits in the back but I cannot seem to get it loaded properly, but then again I have not loaded an instant pack film camera since 1980..

I have attached 2 photo's, can someone tell me what model this is?

I don't know the model number, but that looks exactly like mine, and it takes the Fuji film just fine. Like you, I hadn't loaded a pack film in about 20 years when I got mine and I totally screwed up the first film! I recently purchase Polaroid back and trying to determine what model this is. It has a sticker saying modified in Japan inside.

I think it says "modified in Japan" because the actual Polaroid part of the back was actually made by Polaroid, and Mamyia added the parts that mate it to their camera. I remember mine had some defective parts in the rollers inside after several years use, and I just bought an old Polaroid pack film camera at a flea market and used the parts to fix it. Same innards.
