This is the second time this has happened, both times with Provia 8x10. A sheet of Provia that has the orange color of the base of a negative film and a black and white positive image. That same day, I shot two other sheets from the same box as the offending sheet, which expires October of this year and was opened the day before the shoot in question, and the other two came out fine. I had all three sheets developed at the same lab one day after they were shot. What's going on here? I would think that if the sheet was accidentally cross processed, the sheet would have a negative image. I guess the image is salvageable as a black and white scan.
 
Is there any way to post an example of the affected image?
My first inclination is that someone goofed and used a red or orange safelight to load the film and then realized the error before handling the other sheets.
