I can't remember where I obtained this photo, but as I was doing some 'spring cleaning' on my computer, I rediscovered this photo with a camera that I was curious about. I have some initial guesses on what this camera is, but I'm not exactly sure. Would any of you know the exact name and model of this camera?

Yep, definitely a Pentacon Six (I own two) with waist level finder (interchangeable prism finders are available) . Great camera once the film spacing procedure has been followed, otherwise high chance of overlapped exposures.
The Praktisix did not have film reminder information on the center of the shutter speed dial and film advance lever.

is derived from Pentagonon, a geometric shape with 5 corners
Pentacon
is derived from Pentaprism, a certain prism with 5 sides, and the tradenam Contax, which again likely is derived from the brandname Contessa.
Pentacon was established as manufacturer name in 1964.
