Those look like two differnet purposed cables/ports. The bigger connector for flash synch etc and the smaller 2-prong port for external power. Have you read the manual or do you need a pointer to it?
That looks similar to the Paramount cords version: https://paramountcords.com/product/zsq-3c-metz-45ct1-to-pc-3-foot-coiled/

It appears that the pin arrangement will fit, from the images, but I don't understand the extra notch on the sync cable head?
Does anybody still own a Metz 45 CT-1 and can confirm the sync cable head arrangment?
Hi. Forgive me for (a) not answering your question and (b) seeminlgly questioning the validity of the question, which is not my intent. I see what you see, but what makes you think that after all of these years that there may be a design error or incompatibility in either the synch port or the synch cords, especially since you are looking at Metz products and not aftermarket knockoffs? This might be one of those situations where "analysis pyralyusis" logic is interfering with progress. Please note that I have no first-hand knowledge of Metz products because I eschewed them many years ago due to the proprietary and multiple connectors.
And since someone brought up Paramount... they once were a very honorable and valuable supplier of resonably priced and high quality synch cords. What is seen in the current website is not only rediculously priced but they have been unresponsive to enquiries, which makes me wonder if htey are really in business or not. Sourcing high-quality synch cords these days is near impossible so you should be happy that there is a seemingly ample and affordable supply of Metz cords, should they really fit.

With 6 AA batteries either alkaline or NiMH the 45CT1 should last for a good number of flashes and you can always bring spare. If the OP is using the 45CT1 on a modern camera that only accepts low sync voltage then it's not the flash to use as it can damage the camera. But if the OP is using it on an old camera without dedicated TTL function the Metz 45CT1 would be more convenient to use as it has 5 auto aperture choices. Besides most shoe mount flashes doesn't have the power the 45CT1 has.
Ummm, while I certainly wouldn't dispute a word you wrote I haven't noticed mention of either a shoe-mount flash or an alternative to the Metz the OP already possesses. I'm making hte assumption, rigt or wrong, that the OP either had determined that the Metz meets the need or siply wants to use what is already in posession. Perhaps you have other information that what is in this thread. Interesting and potentially useful information nonetheless.
Sorry Brian. I quoted the wrong post. I meant to quote Eff64 who recommended a bracket and a more modern flash (which I think a shoe mount flash).

Thanks for the reply but I fear you may have fallen victim to "analysis pyralyusis" logic yourself, as you seem to have fabricated some reasons which I never started nor impliedHi. I just asked if anyone knew where I can purchase the correct cable, as the ones I have found listed on the internet stating that they are for this model, do not have matching ports in my opinion.

That could be. It's just that too often folks post on forums like this about how old designs that have been working for decades are somehow and all-of-the-sudden defective. Ebay and many photo stores have that cable(s) specified by the instruction manual. Good luck in your quest.
