Mamiya 7 Closeup kit -- the camera plate

I have a closeup kit for my Mamiya, but I do not have the plate that connects it to the camera. I wonder if one of you with this accessory would be so kind as to scan it or photograph it with a ruler in the picture so that I can make a replacement?
