Leica M diopter. I used it on an M3, not certain if it fits other models. It is a +1.0 strength. Metal, pretty flat, black.

I finally started wearing glasses and haven't needed it. Keep running across it in a drawer and it's time to find it a new home. Give me $3 for shipping (US).
