I've just bought a 250mm Sonnar C on ebay for my 500C/M and after trying it out for the first time this evening I noticed a black 'bar' at the top of the frame. I assumed there was something in front of the lens at first. The bar isn't visible with my 80mm. See attached images. (Apologies for the low quality).

The body was sent to Black on White in the UK for a repair a few months ago, after jamming.

Is the mirror out of place or is this something else?

Long FL length lenses don't allow full frame viewing, the mirror is too short. It's normal.

Very surprised I haven't read anything about this before. I'll have a look at the 500C/M manual...

Well so much for refined compositions with this lens! What an annoyance.

I thought that the "M" in 500C/M was supposed to take care of that issue.
The /M designation identifies cameras with user-changeable focusing screens.

Screens in the earlier non-/M cameras had to be changed by a technician with equipment to set the proper position.

The latest versions of the cameras (503CW et al) have an extended mirror that eliminates the vignetting with long lenses.

This is normal for camera that do not have the Gliding Mirror. I have it with my 503CX. I no longer notice it. Don't worry.
