Grafmatic 45 4x5 film holder in good working condition with 6 septums -- very clean on the inside, the usual paint loss on the outside. Counter works as well.

This is the 1268 model: it will slide into your 4x5 back like a normal film holder (it's three-quarters of an inch deep) OR it can be used with a standard Graflok back.
