I'm not doing too much with medium format these days and have way too much roll film sitting here. All film has been kept since new at near-freezing temp and remain so until shipped. The expiration dates range from 2009 to 2015 (the Provia 100F rolls the most recent), with most in the 2010 - 2013 range. The Ilford PanF+ is the oldest, I think, at 2009. I recently shot a roll of it and it looked great. Of course, with expired film no guarantee, but I've yet to see problems with any of these.
Trying to keep things simple I've arranged the B&W film into three different groups. Hopefully, the inclusion of a much smaller percentage of 220 in a couple of places won't cause a problem but let me know...
