A Mamiya RB67 37mm f4.5 full-frame fisheye on a 4x5 camera. OK, so it's not a fisheye for 35mm cameras, but you can take a 15mm/16mm/17mm full-frame 35mm fisheye, slap it onto a 120 film back and do the same thing:
