Dwayne's order form

When I go to the site and try to open the pdf file for slide processing, my computer says that the file is damaged and won't open it.
Does anyone else have a recent copy they can send to me via e-mail?

I have an old copy, but I wanted a new one because I heard they changed the rates. Can someone tell me the new rates?

You can call or email Dwayne's and they can email it back to you. When I did use Dwayne's some of the things I wanted were not on the form and the form had things that I did not want. They made a form with what I wanted and emailed it to me.

I just looked at several of the files and they all opened great.

One trick to try is to "right click" on the link and save it to disk. THEN open it form there. Sometimes particularly if you use the PDF viewer from Adobe, it get s confused.
