Any idea what went wrong here?

That's 120 Acros obviously, developed in D-76 1+1 in a Paterson tank. No other film in the tank at the time. It's the only frame that seems to have that particular feature.

The tank might have been wet before I started (that was probably the second roll I developed that session), but it would have been through fixing and rinsing since the last development so I wouldn't have expected to get a drip of active dev running down the film.
 
Looks like you got some water on that negative prior to developing the film.

The emulsion swelled where the water was in contact with it, thus accelerating the initial action of the developer in that location.

At least that's my guess.
 
Ah, OK. I thought pre-wetting slowed things down though, not that I've ever tried doing it deliberately.

If not done evenly it can really crap up a negative.
