C41 issue in development

I'm quite new in C41 development. Sometimes I'm getting results like in the attached image. I'm using Tetenal kit. What was wrong? Can anyone give me a hand?

It is sheet film. What temperature did you use?

It's sheet film. Fuji NS160 4x5". I've used 38*C / 100*F.

Looks like incomplete mixing of the chemistry or bad agitation. Not sure. The edge markings seem ok but there is too little shown to really tell.

On closer inspection, I think it might be a film defect.

Is there any good detail on the negative? Scans of my negs look like this when I strongly underexpose them and my scanner desperately tries to compensate. Processing should be fine, since the film rebate looks good ...

One of my mentors told me that I should make sure to properly mix the chemicals before putting them in with the film.

So I always make sure I shake up my bottles right when I take them out of the temp bath and put them in tank/whatever. This is especially important for the bleach/blix.

I can't give you the scientific stuff. I am horrible at all the technical details.
