Any 220 back should have a little higher pressure or closer clearance for the film than a 120 back--I would expect that is where you might experience heavier wear using 120 in a 220 back.
If you are getting 15 exposures, you're probably using a 6x4.5 back rather than the square one. I never see that much extra film in 6x6 on a roll of 120 to cover an extra exposure, certainly not 3 extra exposures. Also, note that clearing the film after the last exposure will require you to dry shoot till you get to the 220 count for the back you have (24 for 6x6 or 30 for 6x4.5).
