This has happed to me on a number of occasions.Remove a partially shot film from a camera.Label it and toss it in the fridge.Attempt to load the film in a new camera and can't retrieve the film leader.
Solution? (I have a Kodak film leader retriever)
 
Next time tape a part of the leader to the outside of the cardridge.
Or was that not the problemm ??

Don't ever fully rewind the film.
I have never rewound film into the canister. If it is a full roll I tear the leaders tongue so the film is square and I know the film is ready for development. If the roll is partial I bend the tongue back and label the frame to start shooting again. My cameras that have auto rewind have the option to leave the leader out. and my manual cameras I can tell when close to the end. If you do wind back in pull the leader immediately

You cannot to that to existing rolls. Being in the fridge helps harden the crimp. So a standard puller may not be effective. A length of fine 'piano' wire may be able to snag a sprocket hole an retrieve.

Err - trying to be vaguely useful, like they say, don't rewind the film all the way; Canon SLRs - some of them at least - have 'don't wind the film all the way in' as a custom function, for what it's worth.
