If the subject is in focus on the ground glass, but in front of the subject when shot on film, how should the ground glass be adjusted? Closer or farther from the lens? (Lets say the lens doesn't suffer from focus shift when stopped down).

The glass would need moving further back.This could occur on a camera back designed specifically for a Glass/fresnel combination where the fresnel is fitted first (lens side) in the back - if the fresnel was missing or removed the focus screen would be out of register.
