I don't know where anyone has seen Foma 400 in 4x5 sheet sizes. Neither B&H, Adorama, Freestyle, or Calumet have it listed, though I'd certainly like to see Freestyle market it under the Arista.EDU Ultra label. I don't even think that such a product exists. Foma itself lists their Action 400 film as available in 35mm and 120 sizes only. No mention of that film in sheet sizes whatsoever. The 100 speed film is listed in sheet sizes and the 200 speed film was until it was discontinued. Whether that discontinuance is permanent or temporary is open to speculation.
