I think there are two pictures here, one (the more disturbing) from just below the nostrils down, and the other from just above the shoulders.

The coating on the cheeks and forehead seems too deliberate to go with the rest, so the message is unclear.

Thanks for the comments. In response, I like contrasts of texture. There isn't a message any more involved than that.
