What a truly wonderful smile. I cannot comment too much on the grain you mention as I do not shoot much B&W at all. I will say the short DOF shows grain in the BG, however, the young lady in the photo appears excellent with quality of an image IMO.

The grain looks fine. Only thing I'd offer is to watch your backgrounds in the future; the sign's a bit distracting. Or maybe it's a halo on your smiling angel.

If you're not against using Photoshop the sign and lights could be removed or minimized quite a bit in brightness. Just a thought to enhance the viewing overall. Regardless, a very nice photo IMO.
