Follow Focus Test 01 : EOS 1000FN --- Both Ladies are UN-SHARP and they were not moving fast -- FAILS !
I think it was 1/250th second as aperture was set to AV and f6.7 -- I think the SIGMA lens motor is not as fast as the genuine CANON one in the ,lens. 
What was your shutter speed? It seems like perhaps the lack of sharpness along every plane in z-space indicates motion blur rather than missed focus.
