What's wrong?

Could anybody advise what went wrong with the sky here? It looks for me like bad development.
The film was developed by Toronto Image Works lab. A reputable place, but I had a number of misfortunes with them recently.

Ah, the rare and jocular cirrus ebulli formation. Very difficult to expose correctly. I like Pete's explanation, perhaps the technician poured his beer in the tank instead of the first fix.
